In order to
participate in the spa rewards program you will need to
sign and date
this form to show you read and understand the rewards program
rules. Your spa dollars do not expire and will continue as long as
Skin Perfection has the rewards program.

every 25 rewards point you will receive a $5.00 credit.

Rewards points do not apply for services rendered prior to the date
